Privacy Practices

St. Luke’s takes privacy seriously and is committed to protecting the privacy of any information that we may create or collect. Information at St. Luke’s is secured by various administrative, physical, and technical safeguards. Examples include: limiting access to information to only those that need the information to perform assigned tasks, locking filing cabinets and offices, and requiring password-protected accounts for electronic systems.

Collection of Information

To best serve our community, St. Luke’s collects a variety of information related to our patients, employees, clients, vendors, and partners. This includes Protected Health Information, Website Information, and other types of information.

Protected Health Information

Protected Health Information is all individually identifiable health information relating to the past, present, or future physical or mental health or condition of an individual; the provision of health care to an individual; or the past, present, or future payment for the provision of health care to an individual. 

St. Luke’s is required under the Health Insurance Portability and Accountability Act of 1996 (HIPAA) to protect the privacy and security of your Protected Health Information. Further information on how St. Luke’s uses and discloses your Protected Health Information or your rights concerning your Protected Health Information can be found in our Joint Notice of Privacy Practices for Medical Information (Joint Notice).

Website Information

Website information is automatically collected to help us improve our Website to provide better service to you. This information is not personally identifiable information about you. Rather, it is information such as the type of browser you are using, the time and date you are visiting the Website, and the pages you visited.

All Other Information

All other information is all information collected by St. Luke’s not covered by our Joint Notice of Privacy Practices for Medical Information or Website Privacy Policy. This may include personally identifiable information unique to you such as your name, social security number, e-mail address, etc. or non-personally identifiable information such as aggregated information, demographic information, etc.

Disclosure to Third Parties

St. Luke’s may disclose information to our third-party vendors to better serve you. St. Luke’s may also, in accordance with law and our Joint Notice of Privacy Practices for Medical Information, disclose your information to third parties to better serve you as a patient. Information disclosures are subject to internal review processes prior to release to third parties.
