Preparing for an Appointment at St. Luke’s Children’s Cardiology

We see our young patients by appointment only. Please notify us if you’ll be late; you may be asked to reschedule. Since paperwork and testing may take a few minutes, we ask that you arrive 15-30 minutes early, especially if you’re new to our clinic. Please notify us at least 24 hours in advance if you will not be able to make your scheduled appointment. This will help us serve all patients in a timely manner.

Please bring your updated insurance card(s) with you to each visit and notify us of any changes in your address, phone number, or insurance information. It may be helpful for you to write down any questions or concerns that you hope to discuss prior to your child's appointment. 

Cardiac medications can sometimes interact with other medications prescribed, so please bring a list of all current medications and the dosages to each appointment, or just bring the medication bottles with you. It’s very important that we know exactly what medications your child is currently taking, and are aware of any medications prescribed by other healthcare providers.
